After you complete this application form and before you press SUBMIT, please print a copy and mail it with the appropriate funds to our post office box to finalize your membership application.  As soon as we receive that correspondence, you will be sent your membership packet and will shortly begin receiving our monthly publication Octagon Topics.   Note that the initiation fee is a one-time charge, and due only will be collected annually.
